Left Back Academy – Liability Waiver & Release of Claims

Please read carefully before participating in any training session.

By attending any training session with Left Back Academy, you acknowledge that you have read, understood, and agree to the terms of this waiver.

By participating in any soccer training, practice, or related activity organized by Left Back Academy, you acknowledge and agree to the following terms:

1. Assumption of Risk

I understand that participating in soccer training sessions involves physical activity and carries inherent risks, including but not limited to injury, illness, collision, strain, or other bodily harm. I voluntarily choose to participate in training provided by Left Back Academy and assume all risks associated with such participation.

2. Release of Liability

In consideration of being allowed to participate in activities organized by Left Back Academy, I hereby release and hold harmless Left Back Academy, its owners, coaches, staff, contractors, and agents from any and all liability, claims, or causes of action arising out of any injury, loss, or damage that may occur during or as a result of training, whether caused by negligence or otherwise, except in cases of gross negligence or intentional misconduct.

3. Medical Authorization

I authorize Left Back Academy staff to obtain emergency medical treatment if necessary and agree to be responsible for any medical expenses incurred. I affirm that I am (or my child is) physically fit to participate in soccer training activities.

4. Media Release

I grant permission to Left Back Academy to use photos or videos taken during training sessions for promotional, instructional, or marketing purposes.

5. Acknowledgment

I have read and fully understand this waiver and release of liability. I voluntarily agree to its terms and understand that this document affects my legal rights. I acknowledge that Left Back Academy is not responsible for any injuries, damages, or losses that may occur during training sessions.
